The religious affairs office has asked for a certified copy of the polygyny permission order. Where can I get it?

Apply at the registry of the Syariah Court that issued the order. Bring your identity card and the case number, and ask about the fee and how long it takes to get a certified copy. If you have a lawyer, your lawyer can usually arrange it. When you receive it, check details such as the spelling of names, identity card numbers and the conditions stated. Small mistakes can delay the marriage arrangements, so ask for a correction promptly if there are any.
