Must the husband bear his wife's medical and childbirth costs?
According to practice and the view of contemporary scholars, basic medical and childbirth costs should be borne by the husband as part of maintenance, although classical scholars differed.
Classical Shafi'i scholars did not include medicine and doctors' fees in obligatory maintenance. However, many contemporary scholars hold that medical care is now a basic need according to custom (uruf), especially childbirth costs. Maintenance is also measured by the husband's means. Consider family insurance or takaful to lighten the burden. See Maintenance (nafkah) and Rights of husband & wife.

Difference of madhhab. All four schools classically did not make medicine and doctors' fees part of obligatory maintenance; many contemporary scholars and fatwa bodies include them on the basis of custom (uruf).
